It looks like a big fat address book, but it's actually a book about you that could end up being your lifeline in an emergency.
It's called the personal medical portfolio, and it was created by a Lubbock woman, Sylvia Farrington. She says after 20 years of working as a nurses aid, she realized patients need easy access to their own medical records. So, she's organized this book, which is sort of a medical diary, for you to keep a record of doctor visits, prescriptions, procedures and much more.
"Sometimes you are nervous in front of a doctor you are scared to ask you are intimidated but if you have it written down and go over it you will be prepared," says Sylvia Farrington.

Sylvia Farrington says she plans to have a kids portfolio available in the near future which will include a shot record.
