Interim "Heart to Heart" Cardiac Program

Interim HealthCare of West Texas' "Heart to Heart" program is designed to provide thorough cardiac home care, teaching, assessment, and coordination of care following a cardiac care clinical pathway.

Our clinical staff; together with physicians, hospitals, and clinics will work with the patient and their families to develop an individualized plan of care that will provide the tools, clinical services and education to promote cardiovascular health, lifestyle modifications and improved outcomes.

Services include:

  • ANTICOAGULANT THERAPY MONITORING (PT/INR)
  • COMPREHENSIVE CARDIOPULMONARY ASSESSMENTS 
  • CLOSE COMMUNICATION AND COORDINATION OF CARE WITH THE PHYSICIAN
  • INDIVIDUALIZED PATIENT/FAMILY EDUCATION
  • INFUSION THERAPY (PERIPHERAL, CENTRAL AND PICC LINES)
  • LAB DRAWS
  • PRE & POST-OP CARDIAC SURGERY/PROCEDURE PREPARATION AND TEACHING
  • CARE FOLLOWING A HEART ATTACK OR EXACERBATION OF CORONARY ARTERY DISEASE PROCESSES
  • PSYCHOSOCIAL SUPPORT
  • RISK FACTOR ASSESSMENT
  • PHYSICAL AND OCCUPATIONAL THERAPY
  • IN-HOME TELEMETRY MONITORING (REAL-TIME)
